How to Qualify for Children’s Rehabilitative Services (CRS)


The state of Arizona has sent out notice that, as of March 20, 2009, they will no longer cover treatment for patients who do not quality for AHCCCS.

 

Those who do not quality for AHCCCS can still receive treatment either through private insurance or by personally paying for their treatment at 100% of AHCCCS rates.

 

AHCCCS applications are available at http://www.azahcccs.gov/Members/HowToApply.asp or call 602-417-4000.

APIPA-CRS will evaluate the request (approximately 3 weeks) and then send a letter requesting the patient’s parents to call to schedule an appointment for the CRS clinic.


1. Download the CRS Referral/Application Form at:

2. Fill out the CRS application form and mail or fax it to APIPA-CRS (instructions are on the form).

3. APIPA-CRS will evaluate the request (approximately 3 weeks) and then send a letter requesting the patient’s parents to call to schedule an appointment for the CRS clinic.
